Error in showing location

Hello.

In my installation the location such as Country, Region, City are became 2 or duplicates on copies in each location. For example, in filter or creating a listing if you select the country the drop down displayed like this...
Country:
Philippines
Philippines

Region:
Region 3
Region 3

City:
Malolos
Malolos
