How to make a number field like this?

Dear Flynax Family,

I need to make a license field for user. The license format is as following
1234-1234-1234

Its 12 numbers in total and there is a (-) for every 4 numbers. Only number. How can I make a number fields like this in adding listing page:
[1234]-[1234]-[1234]
Similar to phone fields like screenshot below

Except it has 3 blank space instead of 2, and each can input 4 digits number.
