Search Form "With Video Only"

Hi Everyone,

Anyone knows how to make the search form with this option "with video only"?
I actually have paid for such customization before more than two years ago. For some reason, I have not saved my customization. When I have contacted Flyanx Support to send me my paid customization, they send me an email telling that I have to pay again for the same customization.

Hello Brian,
Unfortunately we don't save customer customizations for a long period on our side because we do a number of customizations
every day. Probably some one from our team or brave forum user will suggest you the simple solution.
