create form for account type with single-profile

need to add a form to the configured for single-profile account, to insert some more data with the example below:

full Name
address
phone
etc ...

for multi-profile account I saw that it is possible and single profile

Software don't have option "build form" for personal account only for account type.

Hello,

If "single-profile" it's your account type you can add new fields for. Go to Admin Panel >> Account Types find account type where do you want add more fields and click build icon and then add fields from right part to left save and check again.
