Banner plugin 4.10 - image can not be uploaded via front end

My Banners, add a banner, fill in the form but when it comes to uploading of the image, you get a message when you try to go to next step that there are 0 images uploaded and you can not go to next step. There's no indication that you upload an image but if you go back to My Banners, the images is there but status is incomplete as you miss the payment steps.

PS: It would be very nice to be able to use a text banner with a href tags. DS:

I am also haviung the same problem. There is no Upload button in Step 3 (adding new media). Are we missing out on something? There's nothing much in the installation instruction in the plugin page/documentation.
