Average Price Calculation

I am using the "Average Price Calculation" Flynax plugin.

One of the options in the admin panel is "Forms for average price" which has the following options:

1. Category Brower Form
2. Similar ads forms
3. Plugin custom form

Where is the "Plugin custom form"?

Looked but cannot find it.

Here is the path for the admin page:

- Admin Panel > Common > Basic Settings > Average Price Calculation

You can find it in Categories > Categories > your category > Build > Build Listing Form > Average Price Form (button)
